AH day 9

Started the day attending a meeting for all farmers and PT regarding certain workflow changes. -not our biz-

Started packing med after that and mon is back to haunt me with "I am hungry".

After a quick lunch break, Adez and I were attached to the Cardiofarmercy clinic. Its quite nice how certain pt thinks so highly of farmers. In fact, he even said farmers are more impt than dr. LOL. Anyway, was just listening in to the counselling of patients on long term medications that require titration to meet therapeutic outcomes. Like titration of digoxin, statins, antiHTNsives, etc. Already heard 2 pt complaining abt cholestyramine, the infamous bile binding resin. One said that it was bau and the other said its like 喝沙. laughs. What to do, who ask you to have signs of myopathy, increased plasma levels of CK.

Quite inspired to see the gradual progression of the farmer profession. Goot! AH is practising supplementary prescription where farmers are in collaboration with Dr in management of chronic pt, such that farmers can Rx wafarin on their own to titrate doses to achieve goot outcomes.

After clinic, we had to return to office to pick up 1 million stuff to read for case study on O&G. OMG. -roll eyes- And we have to prepare a 5 minute presentation each to the boss at the end of the attachment. And I have to search on counseling points for GTN as well as to do a reverse diagnosis of a long script. Which explains why I am in science library on a saturday morning photocopying koda-kimble and doing my work. Too bad the library is not opened tomorrow. -mugger-

Sometimes I just feel 力不从心, dunno how I am ever going to be as pro as them.

AH day 8

Packed wrong drug twice today and got discovered by the same person. -depressed- Looks for amitriptyline, dothiepin, nortriptyline, fluvoxamine, fluoxetine blah blah. haha.

Anyway, same stuff today. Mon MIA again. Didn't learn much today cause its so freaking busy.

Discovered that frusemide IS furosemide. Feels cheated.

Did my draft layout for display cabinet today. Pending approval. - cross fingers-

Dinnered at AP. -burps-

AH day 7

Woke up at 6am this morning to rush out of house for my CE. Was at my bus stop at 6.45am, amongst the students, the foreign workers and the low wage workers. - yawns -

CE was on genomics. Imagine that. So BT. Watched in amusement as many junior doc fell deep into slumber minutes into the talk. My front row was totally snoozing away except for the female doc directly in front of me. Quite a funny sight. Would have taken a photograph IF there wasn't so many dubious people of unknown positions milling around at the back of the room. Herceptin, transazumab? and aromatin etc are used in the tx of breast cancer depending on the genotype of the cancer tissue, to achieve maximal benefits to risk/ cost. Well, its still all a dreamy research away from it to be translated into actual clinical workflow.

So, the little sleepy bees trudged their tired feet to OP after the CE and stoned. At roll call, Adeline made an announcement and I read the motto of the day. lol. Next time stand at the back. hehe.

Today was crazy omeprazole day. No idea how many $#$#%!^ times I need to pack and top up the ome bin. so -rolls eyes-. I bothered alot of the PT today because I cannot find alot of the weird uncommon drugs. haha. And they are very nice. Ok la, one was super duper friendly, and the other 2 seemed expressionless (but helpful). Tomorrow I shall get to know ALL their names and say hi to all of them! Think PT are quite knowledgeable too!

Mon is on 2 D MC! Grrr, Mon is lazy. ha. Even if I am sick I will still come to work lor. Last yr she also MC at G*******. tsk tsk. See la, mon didn't come which means I didn't learn much. haha. >.<

Was doing PIL at the office in the afternoon for ticlid aka ticlopidine. Was refering to 2 online reference, Uptodate as well as Medscape. Uptodate is only for staff while I already have the Medscape acct. Need to do PIL for spironolactone the next time I see D*reen. Now to think of it, doing PIL at the office is really nice because there is a respite from standing and rushing around at OP.

I didn't study tonight. Didn't know why and how time flew by soooo fast. Its midnight now and my brain don't seem receptive to new info. So off to bed I am.

AH Day 6

Started OP today. It is basically a mad rush left right center to fill prescription. And the sound of "life script" strikes fear in the heart *dengz- dengs!*. Basically, in one day, I think I got a grasp of where all the drugs are placed already. Sometimes the rush is so crazy that it gets mind-numbing, so Mon and I made it a point to record or photocopy certain Rx to review at the end of the day and not just blindingly rushing through from 1 Rx to the next and not learning anything.

And there is the 神 Cl*udine who is some snr farmer who looks v cool and has everything at her fingertips. She demands requests that we come up with 3 learning points to share with her at the end of each day. So, in the midst of crazy packing, we are constantly thinking of special Rx, drug combination or anything special that occurred so that we can share with her and she can explain certain things with us. As usual, AH= learn alot.

*G*******= learn nothing!*

So, I have 6 Rx sitting on my table, with a stack of pdt inserts, and a notebook full of scribbling to go through. And some from last week too. haha. Shouldn't have chionged 太王四神记 the whole wkend. lol. Anyway, can anyone explain the ending to me? I don't understand but Bae did!

Gotta be in AH by 7.30am to catch another CE. -claps- And meet D*reen to discuss my ticlid project, with Sh*k to go through our learning experience over the past week, and finally with 神to go through learning points again. kkz.

Retail therapy for tiny bit. Bought a G2K shirt before coming home. -happy-

Chilling over long weekend

Enjoying the long weekend at home. Before stress sets in again. GoGoGo.

Had an awfully expensive and boring tasting japanese curry rice at AP. The foodcourt at AP is just overpriced and not nice. >.<

Mon brought her 3 jotter books worth of pharmacology to work!

Ha, I look at shit. At least it gives me some discount on my daily dose of coffee qd.

This is the display unit that I am supposed to furnish with stuff relating to DM. Anything I want. All I need to do is to tell the farmer what my idea is and what items that I require. Then I can go around begging for them. haha. Ok, so there is a whole list of possibilities that I can do, but I plan to focus on the lifestyle modification part since there is no point talking about medicine to the patients. There are some low calorie food being sold in the farm which I think i can display. No idea on how to talk about exercise without posting whole lot of words. ha. There is BGM to display but I dunno whether I want those or not. And I want to put up a list of old wives' tales.

Visual visual visual... How. Yawns. Taking alot of sugar doesn't cause diabetes!!!!! One of the misconception that irritates me the most. -roll eyes at dad- HAHAHA.

Oh, I learned that DM pt can still take sugar as long as its part of a balanced diet. Cool, didn't noe that. I wonder who told me they can only take fructose... -looks at a senior-. HAHA.

Anyone wants to suggest anything for the display? Need help. Why am I doing this alone. . .

AH Day 5

Classified intervention data on NH today. Learned more things again. So nice. But more homework. A journal to read on the PK changes in geriatrics.

Can't remember drugs for nuts. After spending a week with Mon, we discovered that although she can't score well in exams, but she remembers a great deal of information and are still able to recall them. On the other hand, I stare blankly at a drug name and can't even say which class it is from but still do better than her in exams. I guess she learn things with passion and get excited over the new info and is able to learn by association, as if she has a mental mindmap. Meanwhile, I learn by organisation and classification, where data are stored in a neat manner such that I am able recall them on a short term basis but retention is very low. =(

Keeps making me wonder whether I am suited to be a farmer. -feels stupid- ha

-streesss- Supposed to set up a display on DM information for the pharmacy as part of my OP project. So stress. Have to think of what to put up and how to put up also. Like I know how... -stress-

haha. Oh well. Learning is good! I think I have worked harder and learned more every single day in AH than I have in school. Spent 5 hours each night working for DI. ha. Can't imagine studying 5 hours during school hours. Mon and I can't figure out why we can't do so in school. We guess there isn't the pressure and expectations to do so unlike at work. Oh well. =)

AH Day 4

Orientation talk by the management for new staff. Rather irrelevant for prec*ptees. Found the CEO extremely inspiring.

Finally got off my ass to buy "the world is flat" and "the undercover economist" at Times after work. Saw CMM, Agnus and Niron at AP!!! Apparently they wanted to shop for working clothes at the G2000 outlet store. Too bad. G2000 outlet store ostracizes guys and only sell short sleeves at like 5% of the floor space while the rest are filled with female clothings. Too bad for niron.

Mon has bipolar disorder. One moment she will be high and the next she will scowl at you. My poor left arm is already badly abused by her and the blue black is 3 cm in diameter la. Poor Ray. HAHAHAHA. lalalala.

Anyway, I remember reading about the bad service of Times bookstore in review done by Life and I am surprised that it REMAINS bad. So sad hor. Lousy management. Cause I was queueing to pay and this Caucasian asked the cashier a question. The cashier replied that she didn't know the answer because she is new here and that is it! Shock leh! She should at least ask her senior or promise to check it out or something la. Because you are new isn't an excuse for customers to be short-changed right? Tsk tsk.

AH Day 3

I will to be brief cause I am tired and have to be there by 8am tmr again. Orientation talk by board for the whole day. Hope it will be interesting!

Today was the last day at DI. After roll call, Mon and I brought Adez to HR to settle her pass but after waiting for a while, the staff suddenly realised that noone was doing the pass after taking the photo. Ended up waiting longer. lol.

Back in the office, we were suddenly dropped a bombshell! She told us that part of her work involve reading a pharmactherapy journal and summarising key points for her farmers. We were then given an article each to summarize. -faints at the journal- Given that we had to hold our discussion after lunch, we had to finish the summary before lunch. -panics- Mine was on the use of high levels of aa in the treatment of chr kidney injury pt undergoing CRRT. -sweats- Too many new jargons and couldn't concentrate hahaha. Mon finished hers first and started TIDUR next to me as I panicked. haha. We were both super hungry but couldn't go until we were done. -glares at mon's shorter and easier paper- Left for lunch after editing the combo PIL.

That mon wanted artery-clogging KFC! LOL. She was super hyper after managing to eat KFC la. And yes, we had to take a walk to AP in the rain to eat her KFC too. One thing I like abt AH is that they provide umbrella at all the umbrella stands! So pro-service right??? I am super amazed la. Just the yesterday, I saw one of the service staff replenishing the umbrella from a box of new ones, which means patients must have taken it and didn't return. Anyway, excellent hospital. Little things that counts. And they are best in the pt waiting time too.

After lunch, we had discussion over the 10 qns. Ha. Realised our preparation wasn't enough. -sobs- I guess our train of thought wasn't that well developed and I think our lack of knowledge is in the way. Oh well, I am glad to say I learned alot! Can't take things for granted anymore! I think one thing I finally got out of 3 days in DI is that even though we had alot of work to do, they were evenly spaced out such that there is never a period of idling at all. (which explains why mon and I think time is passing super slow cf school time)

She took something like 1 hour plus on it and after that, told us to look at all the references and compare between them and look through certain past records etc. When we asked her whether she wants to review acidosis, she said its more for our own studies because she is stressed and rushing for time for P&T. It was then that I so 感动 la haha. I mean she didn't have to spend so much time with us but she did. And I think she is a really nice person because even though we answered alot of things wrong, she didn't scold or anything and told us what were the key issues that she was looking for and she took her time and was always patient with us. So nice right.

Being a DI farmer, one need to be independent but yet be a team player. Even though there isn't the constant rush of OP or IP, there are deadlines to meet and plenty of work to be completed, on top of the normal ward rounds. So time management skills and independence as well as being amazingly capable are important traits.

Oh, I called Liwei today to check on something and darn it, he is overseas la! -freak-
